Online member application won't work

The drop down lists for State, Member Type, and "I will submit my dues by" don't contain any options, thus it won't let me join!

I don't remember how to use US mail.....

Just plug the relevant info into a note within the PayPal page.

Note that dues are increasing $5 per year for all categories on July 1st
