Happened to me on Hell's Revenge. Front wheel, though. We managed to find all five lug nuts, amazingly, four of them within about a foot of each other. I felt cool.
I was cruising my '89 Isuzu pickup on the highway in around 1991 when the passenger rear came off at around 55 mph (pre-national speed limit revision -- "I can't drive... 55!") -- managed to pull over to the shoulder without much issue, but the tire tried to pass on the shoulder just as a buddy was about to get out of the truck... still going 55! Would have killed him and tore the door off if he hadn't seen it at the last minute.
Made a right hand turn in my celica back in HS and when I straightened out my left rear came off. I watched it pass me, hit and curb and fly straight for my neighbors house. Bounced off the gutter, thank god. Even if a tire shop takes a tire off I check them my self.
Someone was talking to Seth after my visit to Crawlertech today. Yeah, rear axle is fine. Pinion is a little deep, but not enough to redo. Solid otherwise. Found lots of slop and movement in both driveshaft splines and in the transfer case / SYE output. Getting prices on driveshaft work and swapping over to a 231 with an SYE.

Btw, I was riding my bike outside of OKC earlier this month when I saw a Dually truck coming in the opposite direction on the interstate lose both wheels on one side. One kept on going down the road, the other attempted to cross a hundred yards in front of me and got caught up in the center fence.
