And I think to a large extent much of what is in this thread is in the interests of not arriving at a similar outcome.

It is not a perfect world and resources are indeed limited.

Kudos to you for your efforts. Condolences for your loss. But it won't help if you try taking out your frustrations on folks who are largely well intended.
