Question for you Navy guys

I would assume the Navy has a "hand reciept"( basically a reciept of equipment issue) form much like the army's DA form 2062.

In the Coast Guard it's know as a DD1149, don't known about the navy though.

Beat me to it. IN ROK we have used DD1149's to tx equipment between services (USMC & USN) also. Prior to going overseas a ton of PPE, helmet, kevlar 782 gear MK-40 & JLISTS were provided to us as individual issue and we signed 1149's.

We always called it a custody card. I never bothered to look at the form number.

NAVSUP Form 306 is the full page version that fits in a binder:

NAVSUP Form 460 is the rolodex card version. I couldn't find an example.

I googled "navy custody card" which is the same thing we use in the corps. The form numbers might be obsolete, but that will get you started.

When we had assigned weapons, we were given the blue custody cards that we turned into the armory when drawing our weps for the watch or patrol. To tx anything including weps between units or cutter to cutter we used the 1149s. Effectively took it off our property list and placed it on the receiving units.

In the Marines we use a Navy form called an Equipment Custody Record, or ECR card. It's NAVMC 10359.

The only thing I remember filling out was the universal request chit, that included the issue national match and high standard 22, that single piece of paper was used from everything from requesting a day off to volunteering for duty in Vietnam or Antarctica Mc Murdo station.
