i've got a 91 4.0 and everything you're describing happened to me about a month back. i replaced a whole lot of parts then finally decided to pull my injectors and clean them. soaked them in some cleaner for about an hour and then used an s.o.s. pad to clean them up. then i took q tips and cleaned the holes and all the connections. ill run out to the garage in the morning and take a pic of the bottle.
