does it use wheel adapter for the big lug dually wheels or did they swap axles?

Wheel adaptors. Metric Ton dually trucks still used the D44 with the adaptors. The wide lug pattern of the dually wheels you see is because the adaptor has a big opening in the center for the lug nuts that bolt it to the 5 on 4.5 lug pattern of the OEM axles. You can kinda see it in this pic:
